Garry Form, born on June 2, 1942, in Runnymede, Saskatchewan, passed away peacefully, surrounded by loved ones, on February 14th. He was the beloved son of Anton and Lydia Formuziewich. Garry's journey through life was marked by dedication, love, and a passion for the great outdoors.


Garry was preceded in death by his first wife, Cheryl, with whom he shared 20 years of marriage, and their son Corey. He is survived by his loving family: his son Daryn Form, along with Nicole, Noah, and James; daughter Kristi Dennison, along with Darren, Rydleigh, Sawyer, and Bryden; and his second wife, Irene, and her daughter Tracy Bond and husband Mark Dekker.


Garry spent a significant portion of his professional life contributing to various fields, including the Canadian National Railway, Imperial Oil, and healthcare, retiring from the Arborfield Special Care Lodge. His passion for travel led him and Irene on countless adventures across Canada, driving from the east to the west coast and exploring as far north as the roads would permit. In his earlier years, he loved snowmobiling and being a pilot...he often mentioned that when he was flying he 'left the world of worries and concerns behind'.
An active member of the community, Garry embraced the camaraderie of curling and participated enthusiastically in various town activities. His love for the outdoors, particularly the Ponderosa, was evident in his commitment to golfing and his role as an engineer for Thunder Rail.


Garry's creative spirit found expression in his back garage, where he could often be found tinkering and welding scrap metal into unique creations. His last years were spent caring for Irene, who now resides in the Arborfield Special Care Home. As illness took hold, Garry found solace and comfort in the care provided by the Nipawin Hospital staff and the compassionate and amazing people at the Arborfield Care Home. Garry's community of friends and neighbors were regularly found visiting with him in his final weeks.


Garry Form took pride in being the first administrator when the Arborfield Special Care Lodge opened its doors in September of 1986, only months after the passing of his first wife, Cheryl. This facility, where he spent his final days, became a poignant final chapter in his remarkable life. Garry's wish for no funeral reflects his desire for a quiet departure, leaving behind memories of a life well-lived and a legacy of love and community.


In lieu of a traditional funeral, the family invites friends and loved ones to celebrate Garry's life by remembering the joy he brought to others and the enduring impact of his kindness. Garry will be dearly missed and forever remembered by all who had the privilege of knowing him. May he rest in peace, reunited with his loved ones, including his wife, Cheryl, son Corey, and infant sons Sean and Lane.
